You will find the answer to your question on page 58 of your 2011 Cadillac & La Salle Club International Membership Directory.   Basically, the procedure is to submit a written request to the national chief judge within 90 days of the meet event.  You should include a self-addressed stamped envelope.  Carl L. Steig is the National Chief Judge and his address is:  2668 Marineview Drive, San Leandro, California, 94577-6837.
