Lassale shifter parts

I have a Hurst shifter for a Lasalle side shift transmission and the shift rods. I need the parts that attach to the transmission for the shift rods attach to. I would like to buy these or see a picture of what they look like so I can try to find or fabricate these parts.

Thanks!! The two small parts in your pictures that slide onto the tranny are what I need. At least now I know what I need. Now I need to figure out how to make them.
